Registration: Adidas Scholarship 
All recipients of the Adidas scholarship must complete the below registration.

Consent Form

I hereby give my son / daughter  permission to play basketball under the guidance of the Impact Youth League - NYC Inc. (herein referred to as "Impact" or "The League").  By signing this document, you agree to adhere to the rules and regulations of The League and its organizers. Impact reserves the right to dismiss anyone from the league if their actions are deemed detrimental or harmful to the league and its willing participants. You understand that all deposit/registration fees are final and non-refundable. 

By signing this document, you certify that your child or children are in good health, with no chronic illness or abnormal tendencies. You understand that Impact's staff, coaches, and organizers are free from any liability for injury to your child resulting from or in any manner connected with your child’s participation in the program. In the unlikely event of an emergency, you authorize Impact's staff to act according to their best judgement in requiring medical attention. If you have knowledge of any physical or mental impairment that would affect your child's participation, please notify all staff members immediately as their participation may not be warranted. 

By signing this document, you hereby waive your rights to bring any action, legal or otherwise against Impact, its staff, coaches or organizers. Impact does not take responsibility for articles of clothing, basketballs, jewelry, sneakers or other personal belongings lost, misplaced, damaged or stolen. You further understand The League retains the right to use for publicity and advertising purposes, photographs and video taken at the game of league participants with no compensation due. 

Purpose of Security Deposit

To encourage full participation during the season, a fully refundable security deposit is required for each player.

Refund Conditions

1. The security deposit is fully refundable and will be returned no later than February 28th of the current season if the player participates in all scheduled games.

   
   

2. Absences due to illness will not count against participation, provided:

   • The parent/guardian notifies the league commissioner at least 2 hours prior to their scheduled game , and

   • A valid doctor’s note is submitted for each sick day within a reasonable time frame.

   
   

Non-Refund Conditions

• 1st Absence: 50% Forfeited

• 2nd Absence: 25% Forfeited

• 3rd Absence: No Refund Issued

Refund Method

Refunds will be issued by check, electronic payment, or the original form of payment by February 28, 2026.
